Initial Visit — $200
Includes a comprehensive review of your health history, focused assessment, and treatment. A personalized plan is discussed and adjusted according to your response.

Follow-Up Visits — $150
Focused treatments tailored to your progress and goals.

Insurance Information

Archetype Acupuncture is a private-pay practice. Payment is due at the time of service.

Veterans Affairs (VA/TriWest Community Care): We accept VA Community Care referrals through TriWest Healthcare Alliance. Authorized visits are fully covered by the VA at no cost to the patient — no copay is required.

To access care, request a referral from your VA provider to Archetype Acupuncture (Sun Yun, LAc), or call (808) 838-6519 for VA assistance. Once authorization is received, our office will contact you to schedule your first appointment.

All Other Insurance Plans: We do not participate with any other insurance networks, including Medicare, HMSA, HMAA, Kaiser Permanente, UnitedHealthcare, UHA, Med-QUEST, Workers’ Compensation, MVA/PIP, and all other commercial plans. Services are provided on a private-pay basis only (with the exception of VA/TriWest). We do not bill or submit claims to insurance companies.

Patients with Medicare or other insurance are welcome to receive care. Upon request, we can provide a superbill — an itemized receipt you may submit independently for potential out-of-network reimbursement. Reimbursement is not guaranteed and is determined solely by your insurance carrier.

Payment & Appointment Policies

We accept the following forms of payment:

• HSA/FSA cards

A 3.6% processing fee applies to all card payments to cover transaction costs.

Credit Card on File
A valid credit card is required to reserve all appointments. The card on file is used only to enforce our missed appointment and late cancellation policy and will not be charged for any other purpose.

Cancellation Policy
We require at least 48 hours’ notice to cancel or reschedule an appointment.
Late cancellations or missed appointments are charged the full appointment fee.
