Initial Visit — $200
Includes a comprehensive review of your health history, a focused assessment, and treatment. A personalized plan is discussed and adjusted based on your response.

Follow-Up Visits — $150
Focused treatments tailored to your progress and goals.

Insurance Information

Archetype Acupuncture is a private-pay practice. Payment is due at the time of service.

Veterans Affairs (VA/TriWest Community Care): We accept VA Community Care referrals through TriWest Healthcare Alliance. Authorized visits are fully covered by the VA at no cost to the patient — no copay required.

• To access care, request a referral from your VA provider to Archetype Acupuncture (Sun Yun, LAc), or call (808) 838-6519 for VA assistance. Once your authorization is received, our office will contact you to schedule your first appointment.

All Other Insurance Plans We do not participate with any other insurance networks, including Medicare, HMSA, HMAA, Kaiser, UnitedHealthcare, Workers' Compensation, auto accident or personal injury claims, and all other commercial plans. Services are provided on a private-pay basis only. We do not bill or submit claims to insurance companies.

• Patients with Medicare or other insurance are welcome to receive care. Upon request, we can provide a superbill — an itemized receipt you may submit to your insurer independently for potential out-of-network reimbursement. Reimbursement is not guaranteed, and all coverage determinations are made by your insurance carrier.

Payment & Appointment Policies

We accept the following forms of payment:

• Cash

• Check

• Venmo

• Credit and debit cards

• HSA/FSA cards

A 3.6% processing fee applies to all card payments to cover transaction costs.

Credit Card on File
A valid credit card is required to reserve all appointments.

• For patients receiving care through the VA, a credit card is still required to schedule appointments. While authorized VA visits are covered and provided at no cost to the patient, the card on file is used only to enforce our missed appointment and late cancellation policy.

Cancellation Policy
We require at least 48 hours’ notice to cancel or reschedule an appointment.
Late cancellations or missed appointments are charged the full appointment fee.
