If you are a veteran and would like to receive acupuncture through the VA, you may request a referral from your VA provider to Archetype Acupuncture (Sun Yun, LAc) or call 808-838-6519 for assistance. Once your referral is approved, we’ll contact you to schedule your first appointment. This service is fully covered by the VA with no copay.

Medicare, HMSA, HMAA, Kaiser, UnitedHealthcare, Cigna, Aetna, Medicaid, Workers’ Compensation, or MVA/auto accident claims (Personal Injury Protection or third-party liability), among others.

Superbill for Out-of-Network Reimbursement

If your plan includes out-of-network acupuncture coverage, we can provide a superbill (an itemized receipt) that you may submit to your insurance for possible reimbursement.

• Archetype Acupuncture is considered out-of-network and does not bill insurance directly.

• To check eligibility, please contact your insurance company to confirm your out-of-network acupuncture benefits and ask how to submit your superbill.

• Please note that any reimbursement is handled solely between you and your insurance provider. We are not involved in the review, approval, or payment process.

• Reimbursement amounts vary depending on your individual plan.
