Initial Visit — $200
Includes a comprehensive review of your health history, a focused assessment, and treatment. A personalized plan is discussed and adjusted based on your response.

Follow-Up Visits — $150
Focused treatments tailored to your progress and goals.

Insurance Information

Archetype Acupuncture is a private-pay practice.

  • Veterans Affairs (VA / TriWest):
    We accept VA/TriWest coverage with valid authorization. Authorized acupuncture visits are covered by the VA and provided at no cost to the patient, in accordance with the referral and authorization on file.

  • Veterans interested in receiving acupuncture through the VA may request a referral from their VA provider to Archetype Acupuncture (Sun Yun, LAc) or call 808-838-6519 (VA) for assistance. Once your referral is approved, we will contact you to schedule your first appointment.

  • All other insurance plans (including Medicare, Kaiser, HMSA, UnitedHealthcare, and other private insurers):
    Services are provided on a private-pay basis. We do not bill insurance companies directly.

Patients with Medicare or other insurance plans are welcome to receive treatment. If requested, we can provide a superbill for your records, which you may choose to submit to your insurance carrier independently. Reimbursement is not guaranteed, and all coverage determinations are made solely by the insurance provider.

Superbills

A superbill is an itemized receipt that includes information some insurance plans require if a patient chooses to submit a claim independently.

  • Superbills are provided upon request.

  • Submission and follow-up with insurance are the patient’s responsibility.

  • Providing a superbill does not guarantee reimbursement.

Payment & Appointment Policies

We accept the following forms of payment:

  • Cash

  • Check

  • Venmo

  • Credit and debit cards

  • HSA/FSA cards

A 3.6% processing fee applies to all card payments to help cover transaction costs.

Credit Card on File
A valid credit card is required to reserve all appointments.

For patients receiving care through the VA, a credit card is still required to schedule appointments. While authorized VA visits are covered and provided at no cost to the patient, the card on file is used only to enforce our missed appointment and late cancellation policy. It will not be used for authorized treatment charges.

Cancellation Policy
We require at least 48 hours’ notice to cancel or reschedule an appointment.
Late cancellations or missed appointments are charged the full appointment fee.
