  Please contact your VA provider directly or online to request a referral for acupuncture services with Archetype Acupuncture (Sun Yun, LAc). Upon receipt of the referral from your provider, we will get in touch with you to schedule your first appointment. The acupuncture service is fully covered by the VA, and there is no associated copay or charge. We take pride in providing support to America's Veterans.

  Workers’ Compensation
  To request a referral for acupuncture services, contact your provider or claims adjuster/case manager. They will transmit the referral to us directly, following which we will contact you to schedule your first appointment. Workers' compensation claims are covered in full by your insurance, with no copay or charge.

• INSURANCES NOT ACCEPTED
  For those with alternative health insurance (such as HMSA, HMAA, Kaiser Permanente, Hawaii Quest, UnitedHealthcare, Cigna, Aetna, Medicare, Medicare Supplement Plans, Medicaid, Personal Injury Protection, Motor Vehicle Accident, out-of-state plans, etc.), we can provide an itemized receipt, known as a superbill, which you may then submit to your insurance company for direct reimbursement, subject to their out-of-network acupuncture coverage policy. As an out-of-network provider, we do not bill your insurance company directly. We recommend you consult with your insurance to determine your coverage. It is important to note that submission of a superbill does not guarantee reimbursement, and the reimbursement process is solely between you and your insurance provider.
